Health Improvement Program



The Health Improvement Program (HIP) is designed to guide patients who are ready to improve their quality of life. With the support of qualified healthcare professionals, patients can learn about the importance of sleep, stress management, mental wellness, self-acceptance, goal setting, fueling the body and enjoyable movement. HPCN and community resources will be shared to support patients in their lifestyle change.

 

 

Health Foundations

Health foundations class is the initial step in the HPCN's all-new Health Improvement Program (HIP). Most people have a pretty good idea how to improve their health. Health foundations looks at what gets in the way. We explore topics of sleep habits, stress, self-acceptance, goal setting, fuelling the body and enjoyable movement. Highland Primary Care Network and community resources will be shared so you can decide how to best manage your care going forward. Even a very small step can create big change.



ABCD's of Digestion (Abdominal Pain, Bloating, Constipation and Diarrhea)

In this two-hour class, you will learn:

  • Information on a variety of digestive complaints
  • Mind-body strategies known to help with digestive issues
  • How diet and lifestyle choices can help you manage constipation, diarrhea, pain, gas and bloating
  • How to make changes to your food intake to improve your food intake and symptoms.

  • Are We There Yet?

    This drop in group offers participants a chance to share their triumphs and work through their challanges. Facilitated by a Highland PCN staff member and featuring rotating short discussion topics such as goal setting, motivation and more.



    Blood Pressure and Cholesterol Essentials

    The Cholesterol and Blood Pressure Essentials Class focuses on reducing your cardiovascular risk by covering the following topics:

  • What is artery disease, hypertension and high cholesterol?
  • What are the risks for these conditions?
  • How do we reduce blood pressure and cholesterol levels?
  • Why is exercise important and what kind is beneficial?
  • Heart Healthy eating and how to eat to lower your blood pressure
  • Reading food labels



  • Craving Change

    Craving Change is Canada's #1 cognitive-behavioral program for people who struggle with their eating habits. The practical, skill-based approach will help you to understand why we eat the way we do. This 3 week workshop (2hr sessions/week.) Please be sure you can attend all 3 weeks. * We do recommend taking the classes Weight Management: Myths and Truths and Mindful Eating prior to attending Craving Change.



    Getting started with Nutrition

    Learn the basics for healthy eating in this class. This class will help with:

  • Learning the basics for healthy eating
  • Gaining an understanding of proper portions
  • Learning how to make our meals balanced
  • Gaining an understanding of food labels
  • Learn the recommendations for exercise

  • Heart Math

    This class uses heart rate variability (HRV) as an indicator of how your nervous system is doing. Learn heart centered breathing and other relaxation techniqes and see your nervous system respond in a positve way in real time using a heart math sensor. *Please note this class has two parts, you can choose to attend only part 1. However you must complete part 1 to be able to attend part 2.



    Intro to Diabetes

    This 3 hour program will focus on:

  • Type 1 and type 2 Diabetes
  • How to avoid complications
  • Food choices, reading labels and dining out
  • Blood sugar testing
  • Learn the recommendations for exercise

  • Mindful Eating

    Learn how to improve your relationship to food and eating by looking at things differently. In Mindful Eating you will learn some introductory steps to part ways with unnecessary food rules and make food fun and enjoyable again.


    Sleep Essentials

    In this class we provides information about sleep facts, and sleep habits. We will look at lifestyle behaviours and how relaxation and cognitive strategies can aid in improving our sleep.



    Slice of Happiness

    In this two-hour class, we will learn what makes people happy and the skills to apply it in our own lives to increase our happiness. This session provides a quick glimpse of what is offered in the four-week "Happiness Basics" workshop. It is not appropriate for those enrolled in the four-week "Happiness Basics" workshop.


    Stressing Well

    This 1.5 hour class will review facts about stress, the impact of stress on our health and steps toward change. This is a discussion focused group where we will talk about strategies to minimize the impact of stress in our lives as well as learn from one another. Some of the material we will cover includes problem-solving, cognitive restructuring, mindfulness, resilience and social support.



    Unpacking Anxiety

    Learn practical skills to help you deal with anxiety.


    Weight Management: The Myths and The Truths

    Feel "weighed down" with your weight loss goals? Leave the weight loss treadmill behind you for good. During this class we focus on the common myths and misconceptions around weight loss. Discover the lifestyle strategies that work to uncover the healthiest version of you.


    What's for Supper?

    Is "what's for supper," a dreaded question in your home? Life is so busy, let's try to make it easier. Join an HPCN registered dietitian to discover some simple strategies to get a balanced dinner on the table. This hands-on class will focus on getting started with meal planning and simplifying those confusing food labels. Feel free to bring your tricky nutrition questions, labels, or favourite family recipes to share and discuss.
